Health Forms Required

Parents are required to complete two medical forms (S865 and MED-1) annually as part of the enrollment process. The forms for your submission include any updated physical examination, immunizations, (students must be up to date unless letter/documentation of exceptions), student health medical /health status, as well as the Med 1 form for medication/treatment in school. Please provide any dental or vision /hearing services provided to the students as well. The forms also include instructions on how medications/equipment are to be labeled when they are to be given during school hours.

Below are the required Physical & Dental Examinaton Forms. Students who DO NOT have the required vaccines may be EXCLUDED from school until they have received the appropriate vaccines or an exemption form has been submitted to the school nurse.

    This handbook contains certain policies and procedures of St. Rose of Lima Catholic School St. Rose Catholic School may change any of its policies and procedures and apply them as circumstances dictate. Families will be notified of major changes through the weekly update or weekly email from the Principal. This handbook may not cover every situation; accordingly, the school principal maintains the discretion to make final determinations regarding how to address situations that arise that are not specifically addressed in this handbook. This handbook will be updated when necessary and reviewed annually by the staff of St. Rose Catholic School and Independence Mission Schools. 

    Parents and students are expected to sign and return the St. Rose of Lima Catholic School Student-Family Handbook Acknowledgement Page.

    Uniforms and Dress Code

    Uniform Policy

    School uniforms are to be ordered through Flynn & O’Hara.

    Neatness and cleanliness in personal attire are very much a part of a child’s education and are the responsibility of both the child and his or her family/guardians. Neat and presentable personal appearance is expected of every student in St. Rose of Lima Catholic School. Students are expected to wear their uniforms on each day of school. If there are extenuating circumstances that affect the child’s ability to wear the uniform, the parent/guardian should contact the school. 

    Throughout the school year, there may be days in which our students may dress out of uniform. Students who do not participate in the “out of uniform day” will be expected to wear their complete uniform throughout the school day. Non-St. Rose of Lima Hoodies are not permitted to be worn during the school day as a part of the students’ school uniform. The teacher will determine where hoodies will be stored in the classroom. Students in “out of uniform attire” that is deemed inappropriate by the administration may be asked to phone a parent/guardian in order to have other clothing brought to school or to be taken home in order to change clothes. Failure to follow these instructions will result in disciplinary action.

    Pre-K – 4 Elementary Uniform

    Prekindergarten 3 and 4 year old boys and girls 

    1. Navy blue sweatpants 
    2. Navy blue shorts 
    3. Navy blue shirt 
    4. Shoes: Sneakers 

    Girls’ Uniform Kindergarten to Grade 4 

    1. Uniform: Blue plaid drop waist jumper 
    2. Blouses: Long or short sleeve blue 
    3. Tie: Red criss cross tie 
    4. Sweater: Navy blue or navy sweater vest 
    5. Socks: Navy blue knee highs or navy tights 
    6. Shoes: Navy blue, black sturdy tie shoes, Mary Jane style, or all black sneakers
      1. Please note, the sole of the sneaker must be black. No colored soles.

      Boys’ Uniform Kindergarten to Grade 4 

      1. Slacks: Heather twill pants (worn on the waist, not on the hips or lower)
      2. Shirts: Long or short sleeve blue golf shirts with SRL logo 
      3. Belt: Black or brown 
      4. Sweater: Navy blue or navy sweater vest 
      5. Socks: Solid colored dark or white socks 
      6. Shoes: Navy blue, black sturdy tie shoes, Mary Jane style, or all black sneakers
          1. Please note, the sole of the sneaker must be black. No colored soles are allowed.

      Gr. 5-8 Middle School Uniform

      GIRLS Uniform Grade 5-8 

      1. Uniform: Blue plaid kick pleat skirt 
      2. Blouses: Long or short sleeve white shirt 
      3. Tie: Red criss cross tie 
      4. Sweater: Navy blue or navy sweater vest 
      5. Socks: Navy blue knee highs or navy tights 
      6. Shoes: Navy blue, black sturdy tie shoes, Mary Jane style, or all black sneakers

      Volunteer Clearances

      We welcome volunteers! To ensure the safety of our students, all volunteers are required to obtain three clearances before serving: PA Criminal Background Check, PA Child Abuse History Clearance, and Fingerprinting. Follow the steps on the IMS Clearances web page.

      Snow Day Procedures

      We will follow the Archdiocese of Philadelphia’s decision on school closures or delays. IF ARCHDIOCESAN SCHOOLS ARE OPEN, our school is OPEN. 

      Our school will:

      • Post message on Class Dojo
      • Send parents text and email messages.
      • Notify KYW1060AM radio, which announces school opening and closures.

       

      TWO-HOUR DELAY ANNOUNCEMENT

      Our school will open at 10:00 a.m.  Breakfast will not be served. Lunch will be served.
